The Incident Information page is where the arresting officer enters details regarding the DWI arrest including: Incident Information, Violations/Charges, Crash Information, Witness Information, Probable Cause, Narrative/Incident Notes and Certification of Arrest.

 

The sections available on the Arrestee Information page are:

• Incident Information
• Violation/Charges
• Crash Information
• Witness Information
• Probable Cause
• Narrative/Incident Notes
• Certification of Arrest

 

On the Incident Information page, the following fields are required:

• Incident Information section:  Date of Incident, Location, City, Parish of Occurrence
• Violation/Charges section:  at least one Violation/Charge
• Probable Cause
• Certification of Arrest
