The Print Forms page is where the user can print a complete Print Package which includes all pertinent form pages for the selected Print Package for:

• Print All – Prints all pages of every form
• Defendant DWI – Prints all pages necessary to give to the Defendant
• DWI Arrest (Officer) – Prints all pages needed for the Arresting Officer
• Defendant Veh Homicide – Prints all Vehicular Homicide form pages to give to the Defendant

In addition to the packages above, the user can print an entire copy of a DWI Arrest form including:

• Uniform DWI Arrest Citation
• Arrestee’s Rights Form
• Intoxilyzer 5000 Checklist
• Uniform Traffic Summons/Complaint Affidavit
• Affidavit of Probable Cause
• Case Officers Document
• Submit DL to OMV Document
• Signature Capture Form
• Vehicular Homicide
• DWI Arrestee’s Handout
• Notice to Withdraw Blood for Chemical Test for Intoxication
• Booking Sheets (for select booking agencies)

 

Print Forms page

 

The officer can choose to print or save as a pdf file a specific Print Package, all forms at once, an entire form, or individual pages of a form for a blank DWI Arrest draft, a partially filled out DWI Arrest draft, or a completed DWI Arrest.

 

To print all forms at once:

1. Click the Select All Forms button at the top of the Print Forms page
2. Click the Print button under the Print Queue to the right.
3. When prompted, take one of the following actions:
   1. Open to view and print:
      1. Click Open
      2. Double click the form to open
      3. Use print function to print form
   2. Save as a pdf file:
      1. Click the arrow next to Save and select the Save As option
      2. Navigate to a location on the computer to save the compressed zip folder
      3. Enter a File name for the pdf file
      4. Click the Save button

       

 

 

To print a specific Print Package:

1. Select the desired Quick Print Form Package
2. Click the Print button under the Print Queue to the right
3. When prompted, take one of the following actions:
   1. Open to view and print:
      1. Click Open
      2. Double click the form to open
      3. Use print function to print form
   2. Save as a pdf file:
      1. Click the arrow next to Save and select the Save As option
      2. Navigate to a location on the computer to save the compressed zip folder
      3. Enter a File name for the pdf file
      4. Click the Save button

 

 To print one entire form:

1. Click the All button under the form name
2. Click the Print button under the Print Queue to the right
3. When prompted, take one of the following actions:
   1. Open form to view and print:
      1. Click Open
      2. Use the browser’s print function to print the form
   2. Save the form as a pdf file:
      1. Click the arrow next to Save and select the Save As option
      2. Navigate to a location on the computer to save the form
      3. Enter a File name for the pdf file
      4. Click the Save button

 

To print an individual page of a form:

1. Click the desired Page number button under the form name
2. Click the Print button under the Print Queue to the right
3. When prompted, take one of the following actions:
   1. Open the page to view and print:
      1. Click Open
      2. Use the browser’s print function to print the page
   2. Save the page as a pdf file:
      1. Click the arrow next to Save and select the Save As option
      2. Navigate to a location on the computer to save the page
      3. Enter a File name for the pdf file
      4. Click the Save button

 

To continue with the DWI Arrest draft, the officer can move to the appropriate page by clicking on a tab in the case menu at the right side of the screen.
