Temporary Driver's License

The Temporary Driver’s License section is where the arresting officer notates if the arrestee’s driver’s license is surrendered and whether a temporary driver’s license is issued.

To enter Temporary Driver’s License information:

  1. To expand the Temporary Driver’s License section, click on the section header or the + in the header bar.
  2. License Surrendered, either: 
    • Select Yes if arrestee driver’s license is surrendered and:
      1. Enter Issue Date of the surrendered license (found on the front of the driver's license)
      2. Enter the Audit Number of the surrendered license (found on the front of the driver's license)
      3. Select whether the driver's license was Destroyed or Submitted to OMV

       

    • Select No if the arrestee's driver's license was not surrendered
  3. Temporary D. L. Issued, either:
    1. Select Yes if the arrestee was issued a Temporary Driver’s License
    2. Select No if the arrestee was not issued a Temporary Driver’s License and select the Reason (Under Suspension, No Driver’s License in Possession, Driver’s License Expired, or Other and enter the reason)

     

  

To continue with the DWI Arrest draft, the officer moves to the Arrestee's Certification of Arrest Signature section.  To expand the Arrestee's Certification of Arrest Signature section, click on the section header or the + symbol in the header bar.
