The Medical Information section is where the arresting officer inputs the arrestee’s answers to the medical questions asked during the arrestee’s interview.

To enter the arrestee’s answers to the medical questions:

1. To expand the Medical Information section, click the section header or the + in the header bar
2. Do you have any physical defects?:  Select Yes or No.  If Yes, enter the subject’s Physical Defect
3. Do you have diabetes?:  Select Yes or No
4. Are you taking insulin?:  Select Yes or No.  If Yes, enter Date and Time of the Last Insulin Dosage
5. Are you hurt?:  Select Yes or No.  If Yes, enter the subject’s Injury
6. Are you ill?:  Select Yes or No.  If Yes, enter the subject’s Illness
7. Have you taken any medications or drugs in the last 24 hours?:  Select Yes or No.  If Yes, enter:
   1. Last Medication Dosage Date and Time
   2. Medication Type
   3. Medication Amount
   4. Medication Reason
8. Have you been to a doctor or dentist recently?:  Select Yes or No.  If Yes, enter:
   1. Doctor Visit Date and Time
   2. Doctor Name
   3. Doctor Visit Reason
