All DWI Arrest Forms (completely blank and unfilled) may be downloaded by any user logged on to the LADRIVING system. These forms are available on the Download Blank Forms page. Support request forms can also be found here. The Download Blank Forms page can be accessed from the Left Side Navigation Menu.

To download a form:

1. Click the “Download Blank Forms” tab in the Left Side Navigation Menu
2. Click the link for the form you wish to download
3. If prompted for a save location, choose the desired location and click the Ok button
4. Navigate to the save location (defaulted to the Downloads folder within your user profile) to open the form
